Reason for appointment (including information about the location of your symptoms (e.g. numbness, pain and/or weakness).
Type and Date (Month/Year) of prior imaging studies including XRs, CT scans or MRIs. You can upload those images here for our office to review.
Description of prior non-surgical treatments including physical therapy, medication or epidurals. For physical therapy, please know the approximate dates of attendance.
Option for International Patients, Those not in Dr. Khormaee’s License Area (NY, CT, CA, NJ) or Those with Emergent Need for A Consultation:
Dr. Khormaee partners with "Best in Class MD" to offer an educational telehealth encounter, outside of your insurance coverage. This may be a good option for out-of-state or international individuals to schedule an educational telehealth encounter using the link to the right. This comprehensive service includes a thorough review of your medical records/imaging followed by recommendations in a report authored by Dr. Khormaee.
The added benefit of this option is that you can schedule this meeting outside of normal office hours and therefore have enhanced access to receive this consultation.
